Affiliated Home Health Care is a leading provider of superior quality home health services serving Southeastern Michigan. We have established successful relationships with most area hospitals, both inside and outside of the Detroit Medical Center (DMC). We service thousands of patients throughout the various healthcare organizations. We work in close conjunction with most area HMOs and insurance companies, as well as the Medicare and Medicaid programs. Our approach to quality is ongoing, and we are committed to being the best in the industry in returning people to the most whole, most functional lives they are capable of after our care. SKILLED NURSING
Our Skilled Nursing services provide professional medical care in the comfort of your home, delivered by licensed Registered Nurses (RNs) and Licensed Practical Nurses (LPNs). Whether you're recovering from surgery, managing a chronic condition, or need short-term care, our skilled nursing team ensures you receive high-quality, compassionate care tailored to your needs.
Our Skilled Nursing Services Include:

Wound care and dressing changes
Chronic disease management (e.g., diabetes, hypertension, COPD)
IV therapy and injections

OUR MEDICATION COMPLIANCE PROGRAM
AHHC's medication compliance program helps patients take their prescribed medications correctly, on time, and at the right dosage, ultimately improving their health outcomes and reducing healthcare costs. Our program includes patient education, counseling, and tools to support medication management. Our Patient Support Programs are designed to educate and engage patients by combining patient insights, and care coordination with health care providers, drive medication adherence and ultimately improve outcomes.
